Transition on family bed to crib

I have a almost one year old and he sleeps with me!! I am losing sleep and can not get him to sleep a full night in his crib. His PED recommended letting him cry it out but after 5 minutes, I couldn't do it and grabbed him and of course laid him down in the middle of me and my husband. He will sleep in his crib for a couple of hours after I feed him but then always wakes up around 1am and thats when he comes into our bed. Not sure what to do. I have tried rocking him after he does wake up and as soon as I lay him back down in his crib he wakes back up, its like he knows I am doing it. Need some advice on what to do now. I am about to start weening him off of breast-feeding and do not want to have this problem when he can't rely on me to put him to sleep after his feeding.
