Just thought I'd share this tip...from my son's Early Intervention specialist!
Put all "emergency" information on a sticker (large shipping labels are good).
List:
Child's name and birthdate
Name and phone numbers for guardians
Nearest relative
Hospital preference
Pediatrician information
Allergies
Medications
Health problems
Then stick the sticker to the back of the child's car seat. This ensures that if you have a wreck the emergency personnel would have pertinent information about your child if you were to be unconscious or killed during the wreck. I thought this was a wonderful idea and thought I should share it.
